New Student Registration Information

If you are unsure of what school your child(ren) will attend, visit the Great Schools website online boundary map to find out which school your child will attend.

To register your child for school, you will need the following items:

● Original official birth certificate w/ seal for your child. (We cannot accept a photocopy.)
● Parent/Guardian Photo ID (with current address in CUSD201 boundaries)

Residency Documentation

● One document from Category I (with current address)
● Two documents from Category II (with current address)

If the custodial parents are not the homeowners or lessees, or your child lives with someone other than his
or her custodial parents, please contact your child’s school office to complete an affidavit of residency. See
the next page for the required documentation.

*CUSD 201 contracts with Thomson Reuters CLEAR, a third-party vendor, to electronically verify student address information. This
service uses parent and/or guardian names and addresses to cross-reference public electronic databases such as utility records,
Department of Motor Vehicle records, and property tax records. Many surrounding school districts use this service to assist in verifying
student residency.

● Photo ID (with current address in CUSD201 boundaries)
● (3) of the following documents from the list below*:

○ Current auto registration/insurance
○ Current medical card
○ Current bank statement or paystub
○ Current cellphone bill
○ Current credit card statement
○ Mail from the State of Illinois

*All with Parent/Guardian name and current address in CUSD201 boundaries

Families may bring residency verification documents to their student’s school. Please contact the school to
arrange a time to stop in.
