Yesenia Trujillo, BSN, RN, PEL-CSN
Westmont High School has a certified school nurse on duty from 7:46 a.m. until 2:46 p.m. The school nurse, under the School code of Illinois, works closely with all staff, utilizing the school health program to contribute significantly to the attainment of optimum health and safety, so that the maximum educational potential of each student is reached. The Nurse’s office is located near the front entrance of the school, next to Dr. Weck's office. The office number is 630-468-8110, FAX number 630-654-2758.
Services include:
- Monitoring and assisting students with everyday health issues, as well as those with more challenging physical needs.
- Administration of medications.
- Maintaining and monitoring student immunization and health records.
- Promoting awareness of health concerns to students and staff.
- Maintaining and monitoring physcian’s orders regarding mental, physical or emotional conditions that may limit student participation in a class and /or extra- curricular activity.
Vision and Hearing screening: Vision and Hearing screening will be provided for Special Education students and foster children that are required by the state to have annual vision and hearing screenings. We also screen students for special education case studies, in addition to students who are referred by a classroom teacher and/or counselor.
Physical Exam/Immunization Requirements: The School Code of Illinois requires each student entering 9th grade to have a physical exam completed by a MD, DO, Advanced Practice Nurse, or Physicians’ Assistant, on file in the health office by the state mandated date of October 15th. The “Medical History” section must be completed by a parent or legal guardian, and the remaining components by the health care provider. The health record must include dates of all required immunizations. Please note for the 2025-2026 school year a Tdap immunization is required for all 9th, 10th, 11th and 12th grade students and any transfer students into those grades as well. In addition, students entering 12th grade must have two doses of meningococcal vaccine, with the second dose administered on or after the child's 16th birthday and at least eight weeks after the first dose. The second dose is not necessary if the first does is administered on or after the child's 16th birthday.
Transfer student requirements: Students transferring from an Illinois school must bring a copy of their 9th grade physical, and complete immunization record. Students transferring from schools out-of-state or country have 30 days from their first day of enrollment to obtain a physical exam, up-to-date immunizations and a Vision exam.
Health Concerns: It is important to identify any health concerns, allergies or medications on the back side of the physical form. Please call the school nurse to identify/discuss any serious chronic health issues that your student may have prior to the start of the school year. Throughout the school year, it is critical to immediately notify the health office of any changes in medical conditions or medications.
Medications: If your child requires medications during the school day, a medication authorization form must be filled out by the parent and prescribing doctor. Students are prohibited from carrying medications or keeping medications in their locker. Only those students who are authorized may carry Epi-pens and/or rescue inhalers.
Thanks for your cooperation!
